Overview Amedisys Hospice RN On-Call Weekend Part-time. Respond to telephone requests at home from our after-hours Triage RN by addressing the request either by telephone or by making an in-person visit that includes skilled assessment and interventions which can include in person after hours admissions and end of life visits. Responsibilities Perform initial nursing assessment on new admissions. Complete, coordinate and revise the total plan of care and maintain continuity of patient care by collaborating with appropriate staff. Regularly communicate patient progress to the clinical manager, physician and care team. Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness and according to care center policies. Record pain/symptom management changes/outcomes as appropriate. Educate families on patient condition, needs and care. Makes referrals to other hospice care disciplines as needed, or document rationale for not doing so. Perform on-call responsibilities and on-call services to patients/families as assigned. Perform other duties as assigned. Qualifications Current RN license, specific to the state(s) you are assigned to work. One+ year of nursing experience required. Current CPR certification. Valid driver's license, reliable transportation, and liability insurance.
