Job Description

We are looking for an empathetic and passionate Home Health LVN to assist a growing home healthcare company. Our patients are home bound and mostly elderly and ill. It is necessary to provide each patient with the time and attention necessary to meet his or her needs.

Notes: The position will be based at Catalyst Health Services, Inc., but will require travel within the city of Houston.
This is a contract position.

 

Recording and updating patient histories.

Discussing health care issues and concerns.

Providing assistance with therapies.

Acting as a point of contact for a patient

Ensures that patients stick to treatment plans.
Monitor patients' vital signs and wound care.
Monitor changes in behavior and progress.
Monitors and administers medication.
Communicates with referring physician.
Participates in professional development activities and maintains professional affiliations.
Performs other related work as required.
