Job Description
This Clinical Pharmacist role focuses on collaborating closely with physicians, nurses, and other healthcare professionals to ensure safe, effective, and cost-conscious prescribing, dispensing, administration, and monitoring of medications. The position requires advanced, current drug knowledge and strong clinical judgment to drive positive patient outcomes beyond those expected in an entry-level clinical pharmacist role. In addition to providing direct clinical services, the pharmacist participates in educational programs, supports basic and advanced pharmacy department services, and performs all responsibilities associated with a Clinical Pharmacist I, including drug distribution, supervision of support staff, and non-dispensing functions.
Responsibilities
- Evaluate medication orders for accuracy, completeness, and therapeutic appropriateness, resolve discrepancies through direct communication with prescribers, clinical pharmacists, or nursing staff, and dispense medications accordingly.
- Assess the feasibility of extemporaneous compounding, perform or verify compounding of specialized products, and recommend appropriate alternative products when needed.
- Prepare IV admixtures and compound sterile products, including large-volume parenteral solutions, mini-bags, total parenteral nutrition (TPN) solutions, and hazardous agents such as cytotoxic drugs, following all safety and quality standards.
- Review patient profiles to identify contraindications and potential drug interactions, and screen medications and dosages to ensure appropriateness based on therapeutic intent and the patient’s physiological status.
- Participate in monitoring drug therapy under the supervision of a senior clinical pharmacist when working in decentralized settings, ensuring continuity of care and optimal medication management.
- Dispense controlled substances and maintain a perpetual inventory, consistently adhering to all legal requirements, regulatory standards, and departmental policies and procedures.
- Provide functional supervision to pharmacy support personnel, including monitoring attendance and work quality, organizing and prioritizing resources according to workload, and directing workflow to meet departmental standards and time frames.
- Enter, verify, and maintain accurate computer records and data for medication orders, ensuring precise documentation and system integrity.
- In decentralized settings, instruct patients and families on the safe use of medications, providing counseling on dosing schedules, administration techniques, and potential adverse effects.
- Monitor drug therapy for assigned patients and take appropriate action to optimize drug selection, dosing, and safety, minimizing adverse reactions and ensuring medications align with therapeutic intent and the patient’s physiologic functions.
- Participate independently in patient care rounds, review patient-specific data to evaluate clinical status and medication needs, and develop, implement, and adjust therapeutic plans.
- Counsel and educate patients regarding their medications, including correcting dosing schedules, identifying and addressing duplicate therapies, and reinforcing adherence and understanding.
- Respond to both evaluative and non-evaluative drug information requests from healthcare professionals and patients, providing evidence-based, clear, and timely information.
- Promote compliance with clinical practice guidelines, formulary policies, and prescription benefit management procedures, reducing non-formulary use and therapeutic duplication, and recommending appropriate alternative medications and dosage adjustments.
- Provide educational services and professional development support to pharmacy technicians, clinical pharmacists, pharmacy residents, nurses, prescribers, and other healthcare professionals.
- Maintain current knowledge of trends, guidelines, and developments in pharmacy practice by reading professional literature, attending seminars and conferences, and completing required continuing education credits.
- Contribute to departmental and organizational goals and objectives by adhering to policies, procedures, quality standards, and safety standards, and complying with all applicable governmental and accreditation regulations.
- Participate in meetings and committees, representing the pharmacy department and supporting hospital initiatives, including involvement in community outreach activities as appropriate.
- Engage in multidisciplinary quality and service improvement teams to enhance patient care, medication safety, and overall pharmacy services.
Essential Skills
- Doctor of Pharmacy (PharmD) degree.
- Active pharmacist license in the District of Columbia.
- 1–3 years of direct patient care experience in a hospital setting.
- Demonstrated oncology pharmacy experience (required).
- Strong clinical knowledge of oncology medications and related therapeutic regimens.
- Proficiency in sterile IV compounding, including preparation of IV admixtures and hazardous agents.
- Ability to evaluate complex medication orders for accuracy, safety, and cost-effectiveness.
- Effective collaboration and communication skills with physicians, nurses, and other healthcare professionals.
- Capability to supervise and direct pharmacy support personnel in a busy clinical environment.
- Competence in using pharmacy computer systems for order entry, verification, and documentation.
Additional Skills & Qualifications
- Experience with Investigational Drug Service (IDS) operations is preferred but not required.
- Background in hospital pharmacy practice, including IV compounding and handling of cytotoxic drugs.
- Strong patient counseling skills, with the ability to explain medication regimens and potential adverse effects clearly.
- Commitment to ongoing professional development through continuing education, seminars, and literature review.
- Ability to participate effectively in multidisciplinary rounds, committees, and quality improvement initiatives.
- Attention to detail and adherence to regulatory, accreditation, and departmental standards.
Work Environment
This position is based in a hospital pharmacy setting with a focus on oncology and sterile IV compounding. The standard schedule is Monday through Friday from 8:00 a.m. to 4:30 p.m., with no weekend shifts required. The role involves working in clinical and decentralized pharmacy areas, participating in patient care rounds, and collaborating closely with multidisciplinary teams. Pharmacists routinely use pharmacy information systems, compounding equipment, and safety protocols for handling hazardous and cytotoxic medications. The environment emphasizes high-quality patient care, compliance with regulatory and accreditation standards, and active involvement in education, quality improvement, and community outreach.

Pay and BenefitsThe pay range for this position is $64.00 - $80.00/hr.
