Registered Nurse Hospice

Job Summary:
The Hospice Nurse provides comprehensive in-home hospice care to patients, focusing on comfort, symptom management, and support for patients and their families. They collaborate with a multidisciplinary team to develop and implement individualized care plans and ensure quality standards are met.

Responsibilities:

• Assess patients’ physical, psychosocial, environmental, safety, and developmental needs upon admission and during follow-up visits.
• Develop personalized care plans based on comprehensive assessments and update documentation accordingly.
• Coordinate care activities with healthcare team members to optimize patient comfort and safety.
• Provide education and emotional support to patients and family members regarding hospice care.
• Participate in quality assessment and Performance Improvement programs to enhance service delivery.
• Conduct follow-up visits to monitor patient progress and adjust interventions as needed.

Qualifications:

• Hold a valid Registered Nurse (RN) license.
• Demonstrate at least one year of Hospice care experience.
• Possess strong assessment and care planning skills.
• Exhibit excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of hospice regulations, safety protocols, and patient advocacy.
