Registered Nurse Case Manager

Job Summary:

The Registered Nurse - Case Manager provides skilled nursing care and case management services in an acute hospital setting. This role involves patient assessments, medication administration, treatment planning, and collaboration with interdisciplinary teams to ensure high-quality patient outcomes. The nurse will work primarily day shifts on a 4x10 schedule, supporting acute care patients and utilizing Epic electronic health records. Strong communication and acute care experience are essential. Responsibilities:

• Perform comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments according to physician orders and protocols.
• Coordinate patient care with interdisciplinary teams, including physicians, therapists, and social workers.
• Utilize Epic electronic health record system for documentation and communication.
• Manage case load and provide education and support to patients and families.
• Comply with hospital policies and maintain accurate clinical records.
• Float within nursing scope to meet facility needs as required.
• Work every other weekend or 4 weekend days within a 4-week schedule.

Required Skills & Certifications:

• Current Alaska Registered Nurse (RN) license in hand.
• Basic Life Support (BLS) certification.
• At least 1 year of acute care nursing experience.
• Experience with Epic electronic health records system.
• Strong clinical assessment and treatment planning skills.
• Ability to work effectively in a fast-paced acute care environment.

Preferred Skills & Certifications:

• Accredited Case Manager (ACM) certification.

Special Considerations:

• Shift is 4 days per week, 10 hours each day (4x10 schedule).
• No on-call requirement.
• Weekend work required every other weekend or 4 weekend days per 4-week schedule.
• Only one time-off request allowed per 13-week assignment (up to 5 days).
• No holiday time-off allowed for auto offer candidates.
• Floating within nursing scope to meet facility needs is expected.
• Candidate's permanent address must be at least 50 miles from the facility; locals not accepted.

Scheduling:

• Work schedule consists of 4 consecutive 10-hour day shifts per week.
• Guaranteed 70 hours per 2-week pay period, with possible call-off for one 10-hour shift every 2 weeks.
• Shift times may vary but will fall within the day shift range.
