Licensed Practical Nurse (LPN)Enterprise Dr - State College, PA 16801OverviewPosition Type Full Time Education Level Certification Travel Percentage Road Warrior Category NurseDescriptionPosition SummaryThis individual must be willing to assist with nursing functions for individuals with disabilities, co-existing medical and mental health conditions. This role will help ensure the individuals' health, safety, and welfare, while building relationships and encouraging community inclusion allowing them to experience life to its fullest. This position requires an individual who is flexible, a team player, takes direction, highly organized and efficient.Essential Job FunctionsCompletes health appraisal when consumer is entered into the programProvides assistance for interactions with appropriate medical professionals as necessaryAttends all discharge meetings from hospitals, rehabilitation centers, etc. for planning and follow-up, with Program SpecialistMay be required to provide direct care and complete work shifts at the nursing pay rate based on program and consumer needsAs necessary, assists consumer and staff with any treatments prescribed by physician or any other medical professionalEvaluates, observes, and assesses physical health conditions and when necessary, documents informationPerforms monthly routine non-evasive health examinationsTrains staff to correctly follow through with physicians' ordersAssist in providing training or developing training programs for all staff of individuals when necessaryObtain Medication Administration Trainer Certification per Office of Developmental Programs Train the Trainer course in order to train staff in Medication Administration Procedures. Assist in training staff as neededAssists in administering, or monitoring administration of, medications. Follows ODP guidelines for correct Medication Administration ProceduresMonitor consumers' medication administration as needed, to ensure medication is being administered as prescribedComplete a monthly medication review checklist for all ID program consumers and assists with MH program consumers as neededCompletes and facilitates accurate medication disposal proceduresAssist with requesting medication refills, weekly and as needed pill counts, and discussing medication with medical professionals and pharmacistsCoordinate arrangements for medical equipment as necessary and/or as prescribedMonitors vitalsMonitors intakes and discharges when necessaryEducates consumers on medication, physical health, psychiatric health, nutrition, and self-careEducates staff on any needed physical health matter for consistency in careWorks with the ID management team to complete the consumer's Health Risk Screening Tool (HRST) clinical ratings and responds to reviewsContinually coordinates care with pharmacy, physicians, and other health care providersParticipates in treatment team and ISP meetings as necessaryProvides medication/health teachingPromotes healthy practice in all aspects of conduct of lifeInteracts with families, medical providers, other agencies, and any other designated personnel in a positive and professional mannerAssures the protection of all individuals from abuse and neglect; provides an environment that promotes general health and when applicable, healing from injury or illness; obtains assistance from medical personnel as needed; documents results of these individual contacts and notifies appropriate personnel as applicableResponds to incidents in accordance with regulations/policyUnderstands and respects individual rights; encourages individuals to exercise their rights; encourages and enhances personal decision making while setting aside personal bias. This may include providing direct care work shifts.Must have a valid driver's license, good driving record, and access to a reliable vehicleCertified in medication administration and be a certified trainerMental and Visual DemandAbility to read, write, and speak English and follow written and verbal instructionsMust be highly skilled in dealing with people, particularly with Intellectual DisabilitiesBe able to write and/or print legiblyMust exhibit writing skills required for medical support of the ID departmentAbility to be mentally and visually alert at all timesAbility to work independently as well as in groups and one on oneFlexibility and initiative to take on new tasks/projectsAbility to be attentive to detailsAbility to coordinate simultaneous operationsAbility to exercise manual coordinationMust exhibit excellent verbal skills to interact with consumers and staffPhysical DemandMust possess ability to exert moderate physical activityMust possess the physical stamina to deal with medical emergenciesDue to the nature of our individuals' disabilities, must be able to implement emergency restraint procedures and first aid (requires ambulation and use of all limbs)Physical stamina and emotional stability to be able to deal with medical and psychiatric emergenciesWorking ConditionsDuties are carried out within the Centre County community, between our different sites and the main office.Blood Borne PathogensThe normal work routine may involve exposure to blood, body fluids or tissue. Exposure or potential exposure may be due to the possibility of a need arising to perform emergency first aid on an individual.Unavoidable HazardsOccasionally subject to aggressive behavior from individuals.
