Key Responsibilities

• Assess and evaluate patients' physical conditions upon initial visit to establish comprehensive treatment plans.
• Utilize varied therapeutic methods including exercise, stretching, hands-on therapies, and specific equipment to alleviate pain and promote mobility.
• Develop individualized treatment plans aimed at helping clients achieve maximum functional independence.
• Document patient progress meticulously, including responses to treatment and modifications to care strategies.
• Continuously evaluate patient goals and progress, readjusting treatment plans as needed to ensure optimal recovery.
• Educate clients, families, and caregivers on therapeutic exercises and physical therapy tools to foster self-management and independence.
• Stay informed on the latest techniques and industry standards by attending workshops, engaging in professional networks, and participating in training programs.

Patient Population / Client Focus

Your primary focus will be on a diverse patient population, including children and adults facing recovery challenges from injuries, surgeries, or chronic conditions. By providing individualized care, you will play a crucial role in their rehabilitation process, promoting long-term health and well-being.

Qualifications

• Current Doctorate in Physical Therapy (DPT) and state licensure as a physical therapist are required.
• Strong written and verbal communication skills to effectively convey treatment plans and progress to clients and families.

• Ability to demonstrate flexibility and adaptability in dynamic home care settings.

Preferred Experience
