Job Title: Wound Care Nurse Practitioner (AGNP/CNP) | On-Site Senior Care .

Company: Twin Cities Physicians.

Compensation: $105,000 to $120,000 annually .

Benefits: Full Benefits Package (FT status only) .

Location: Greater Twin Cities Metro Area, MN.

Schedule: Full-Time or Part-Time .

Position Summary:

As a Wound Care focused Nurse Practitioner , you will round within senior communities, delivering hands-on care and managing wound treatment plans. This role offers autonomy, flexibility, and the ability to build long-term patient relationships while making a measurable impact on outcomes.

Responsibilities:

• Provide on-site rounding and wound care services .

• Manage treatment plans, medications, and labs .

• Collaborate with physicians and care teams .

• Communicate with families and staff .

Qualifications:

• Active Certified Nurse Practitioner (CNP) license in Minnesota .

• Experience in geriatrics, primary care, or wound care preferred .

• Valid driver’s license.

• Strong passion for senior care .
