Job Description
Title: Registered Nurse - Home Health

Schedule: 8-hour shifts

Assignment Details:
Contract Length: 14 weeks
Guaranteed Hours: 40 hours per week

Requirements:
- Current state license as a Registered Nurse
- CPR certification
- 1-2 years of experience as a Registered Nurse in a clinical care setting
- 2+ years experience in home health
- Homecare Homebase Software experience
- OASIS experience
- CHPN certification (preferred)
- Reliable transportation and insurance

Roles & Responsibilities:
- Assess patients in home health and/or hospice to identify their physical and psychosocial needs.
- Develop, implement, and evaluate the Plan of Care (POC) to ensure quality care.
- Educate patients and families about care, medications, and safety practices.
- Supervise and provide clinical direction to home health aides to ensure quality service.
- Monitor cases for compliance with third-party payer requirements.
- Communicate effectively with patients, families, and healthcare team members.
