What You’ll Do Manage a home health caseload across homes and community settings . Complete OASIS assessments and individualized care plans . Provide skilled nursing care, education, and coordination . Adjust care plans based on patient progress and needs . Collaborate with physicians and interdisciplinary teams. Maintain accurate, timely documentation . What We’re Looking For 3+ years of RN experience, home health preferred. Strong clinical judgment and care planning skills. Ability to work independently in a field-based role.
