Job Title: Physical Therapist (Home Health) Location: Morgan City LA,70380 Coverage Area: Service area includes, but is not limited to: Jeanerette (west) to Gibson (Bayou Black Rd area) Charenton, Baldwin, Franklin, Centerville, Ricohoc Patterson, Berwick, Morgan City, Stephensville Pierre Part, Amelia, Bayou Lourse Duration: 3 months contract with possible extensions Schedule: Monday – Friday, 8:00 AM – 4:30 PM 40 hours/week (8-hour shifts) Position Summary: We are seeking a dedicated Physical Therapist (PT) to join our home health team. This role involves providing patient-centered physical therapy services across a defined service area, focusing on evaluations, treatment planning, and ongoing care in a home-based setting. The ideal candidate will have strong home health experience and be comfortable working independently while managing a dynamic patient caseload. Key Responsibilities: Conduct comprehensive physical therapy evaluations for home health patients Develop and implement individualized Plans of Care (POC) Provide therapeutic treatments aligned with patient goals and physician orders Accurately complete and maintain OASIS documentation Monitor patient progress and adjust care plans as needed Educate patients and caregivers on treatment plans, exercises, and safety Ensure compliance with all regulatory and clinical guidelines Collaborate with interdisciplinary healthcare team members On-Call / Weekend Requirements: Occasional availability preferred for PT evaluations , if needed Qualifications (Required): Active Louisiana Physical Therapist (PT) License Prior Home Health experience Experience with Homecare Homebase (HCHB) required Strong clinical assessment and documentation skills Ability to work independently and manage travel across service areas Preferred Qualifications: Extensive Home Health and HCHB experience Strong communication and patient education skills Adaptability and time-management skills in a field-based environment Compensation & Benefits: Mileage reimbursement between patient visits (first to last visit): Yes Competitive travel assignment compensation (details provided upon inquiry) Dress Code: Navy scrubs preferred , though any clean and professional attire is acceptable
