Afterhours Hospice RN Care Manager

SouthernCare is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will assess needs, coordinate care, and maintain clinical oversight for assigned patients during nights, weekends, and on-call shifts. You will work with the Interdisciplinary Group to ensure alignment with physicians and families. Active licensure, ongoing education, and strong documentation are essential. #J-18808-Ljbffr
