Nursing Assistant/Resident Assistant (CNA) Part-Time Overnights Located in Rushford, MN Hours: Non-Exempt/As Scheduled Reports to: Director of Nursing Travel: Some travel may be required. Job Summary: To provide direct resident care services according to the plan of care prepared by and under direct supervision of the Charge Nurse. Work Environment: Works in well lighted well-ventilated work area. Essential Functions:
• Provide direct resident care services with attention directed toward improving or maintaining optimal physical, mental, and psychosocial functional performance, promoting resident choice and preferences, and increasing resident dignity and quality of life according to each resident's plan of care and the NA Standards of Care. 1. Nutritional care, including: a. Asking resident their favorite food and beverage choices. b. Serving resident meal and snacks with correct diet and consistent in a timely manner to ensure proper temperature. c. Assisting, cuing, or monitoring residents with eating d. Encourage adequate intake as directed. e. Recording intake as directed. f. Supplying fresh water to residents during each shift. g. Measuring weight for resident as directed. h. Watching for and reporting to nurse changes in resident appetite, intake, and ability to eat. 2. Continence care, including: a. Supporting use of bedpan, urinal, commode, or toilet as appropriate. b. Following established bowel and bladder program. c. Collecting routine urine, stool and sputum samples as directed. d. Keeping urinary catheter and tubing properly positioned and cleaned. e. Emptying and recording output for each shift as directed. f. Providing colostomy care as directed, observing for results as directed. g. Recording bowel movements and character as directed. h. Keeping incontinent residents dry and clean. i. Cleansing after each incontinent voiding or bowel movement j. Watching for and reporting to Charge Nurse changes in resident elimination patterns 3. Mobility assistance, including: a. Assisting with ambulation to the extent needed b. Using walker, cane, wheelchair, gait belt and other mechanical aids according to the care plan c. Transferring resident between bed, wheelchair, geriatric chair, or commode using assistive devices, gait belt and proper body mechanics d. Maintaining optimum joint functioning for activities of daily living by positioning using proper body alignment e. Moving and regularly repositioning residents who are lying in bed f. Unser proper body mechanics and turning techniques g. Encouraging residents to be up and as active as possible h. Conducting range of motion exercises as directed i. Assisting with rehabilitation and restorative programs and goals according to each resident's care plan j. Encouraging resident participation in therapeutic activities k. Assisting residents to and from social and therapeutic activities as direct l. Watching for and reporting to Charge Nurse changes in resident mobility patterns or safety m. Reporting incidents or injuries to the Charge Nurse immediately 4. Personal hygiene cares, including: a. Assisting with bathing-bed, whirlpool, or shower b. Assisting with shampooing and hair care c. Assisting with skin care needs d. Assisting with grooming and perineal care e. Assisting with dressing and undressing f. Assisting with oral and denture hygiene g. Assisting with nail care h. Assisting with shaving i. Watching for and reporting to nurse changes in resident ADL abilities or skin integrity 5. Comfort and quality of line needs, including: a. With attention directed toward embracing the Culture Change philosophy, make decisions and carry out duties according to the resident's preferred routines and choices. b. Assisting the resident in meeting the needs for uninterrupted rest and naps c. Watching for signs of discomfort and reporting to the nurse immediately d. Reporting resident complaints of discomfort to the nurse immediately e. Following the resident's care plan choices.
f. Assisting with socialization, exercise, and mental stimulation of the resident g. Maintaining a clean, comfortable, and pleasant environment h. Changing bed linens and making beds i. Tidying the resident's room j. Observing of proper ventilation, temperature, light, and noise control 6. Effective communication with residents, families, and other staff by: a. Communicating therapeutically and respectfully with residents by using terminology, manner of speech and non-verbal techniques appropriate to the individual's level of understanding. b. Providing adequate compensation for diminished hearing. c. Establishing eye contact and speaking directly to residents. d. Using low decibel sound levels. e. Decreasing extraneous noise during conversation. f. Responding to resident's verbal and non-verbal expression of needs g. Reporting to nurse if a request cannot be met h. Receiving shift report from the nurse at the beginning of each shift i. Communicating professionally between staff for continuity of care and safety. j. Reporting off to the nurse before leaving duty. k. Watching for and reporting to the nurse and changes in resident usual communication pattern immediately. 7. Providing for resident, employee and GSLS protection by: a. Practicing basic medical asepsis to prevent the transfer of infection -using correct hand-washing techniques -handling supplies and equipment with proper technique, moving cleanest to dirtiest -cleaning and sanitizing bedside equipment and supply as assigned b. Positioning call light in reach of residents always, use of protective devices according to the resident's plan of care, observing the "No Walk By" call light policy and responding to call lights promptly, observing the "Catch a Falling Star" Fall Prevention program, and other safety measures, policies, and programs. c. Knowledge and practice of HIPAA privacy regulations d. Providing adequate compensation for impaired visual and hearing e. Reporting potential hazards, incidents or injuries to the shift nurse or administration immediately f. Observing self-back safety and body mechanics procedures g. Knowledge and following each resident's plan of care unless resident status has changed, then seek direction from the shift nurse or Nurse Case Manager. Always seek additional help for resident cares when resident status warrants it. h. Knowledge and practice of all established policies and procedures of the GSLS i. Knowledge and practice of all established emergency and disaster plans j. Knowledge, thorough understanding, and proper performance of job procedures, skills, proper body mechanics, individual resident care plans, and equipment use prior to performing job duties k. Awareness of resident, staff, and public safety always l. Providing for resident protection by: -using special need safety devices -assisting or supervising residents with smoking -adjusting the bed height to the lowest position with brakes on -observing residents at least every two hours or more frequently for special needs m. Reporting as a mandated reporter any vulnerable adult issues n. Watching for and reporting to the nurse changes in the resident's physical status, mental status, and behavior immediately 8. Other Skills (with special training), including: a. Taking vital signs b. Changing oxygen tanks c. Documentation of care provided, vital signs, intakes and outputs, behaviors and others as directed d. Other duties as assigned by the licensed nurse or Household Manager Skills and Abilities:

• Good Observation Skills-remains constantly alert to observe changes in resident's status, recognized problems and hazards and report to the appropriate person. Uses observation skills prior to providing cares to determine if a licensed nurse needs to assess the resident.
• Ability to react calmly and efficiently to new or stressful situations.
• Demonstrates understanding and support of management- communicates objective information to nursing management, demonstrates support of management to staff, family, and the community.
Physical Demands:
• Regularly required to stand, bend, and walk. May also be required to sit, climb, stoop, kneel, crouch, push and pull.
• Regularly assigned to lift or move objects up to 10 pounds.
• Frequently required to support up to 10-50 pounds.
• Emergency situations could require carrying/supporting 25 pounds or greater.
• Requires employee use hands for repetitive actions such as simple grasping, firm grasping and fine manipulating.
• May be required to work in unprotected heights, being around moving machinery, exposure to marked changes in temperature and/or humidity, driving automotive equipment and exposure to dust, fumes, or gases.
• Requires functional hearing ability to read medical records, make observatory assessments, and document in writing and computer records.

• Must be at least 16 years of age.
