Per Diem Mail Order Long Term Care Staff Pharmacist in Horsham, PA

Per Diem Staff Pharmacist Long Term Care & Mail Order Location: Horsham, PA Schedule: Days, Monday to Friday, 7:00 AM 3:30 PM, every other weekend Duration: 8-week contract This role offers the opportunity to apply your pharmacy expertise in a focused long-term care and mail order setting. You will ensure patients receive safe and effective medication while contributing to the smooth operation of pharmacy services. Key Responsibilities: Accurately accept, dispense, and validate prescription medications Monitor patient medication adherence and identify any patterns of over-usage Ensure compliance with legal regulations regarding medication supply Assess appropriateness of prescribed medications based on patient needs Maintain confidential patient records and manage pharmacy documentation Counsel patients on medication use, potential side effects, and answer their questions Provide consultations on prescription and OTC medications, including drug interactions and dosage Support patient health programs such as smoking cessation, blood pressure monitoring, and cholesterol management Educate patients on using medical devices, e.g., blood pressure monitors and glucose meters Compound medications and prepare sterile solutions following established protocols Supervise pharmacy technicians and clerks to maintain operational efficiency Manage inventory levels of pharmaceutical and medical supplies Collaborate with healthcare providers to address drug-related problems and optimize patient care Qualifications and Experience: Pharmacy experience with a strong foundation in dispensing and patient counseling Experience in long-term care and/or mail order pharmacy preferred This per diem assignment offers a balanced schedule with rewarding clinical responsibilities in a supportive environment.
