Lawrence County Memorial Hospital seeks a dedicated Swing Bed Coordinator / Case Manager to lead admissions reviews and discharge planning. You will collaborate with ED, physicians, and the care team to determine level of care, coordinate post-acute referrals, and ensure accurate documentation and payer communication.
