RN Admissions Nurse - Polk County, FL

Compassionate Care Hospice (CCH), an Amedisys Company, is seeking an RN Admissions Nurse based in Polk County, FL. This position requires weekday travel to assigned areas. Schedule: Monday-Thursday, 4-10's. Required: Prior Admissions experience in a Hospice or home health setting. At CCH, we go above and beyondnot only for our patients but for each other. Salary Range: $82,000$87,000 base, plus bonus opportunities. Final compensation may vary based on experience and skills.

Responsibilities

• Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
• Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
• Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
• Functions as a liaison between disciplines and facilities during the admission process.
• Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
• Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

Required:

• Prior Admissions experience in a Hospice or home health setting
• Current RN license in the state(s) where you will work
• Minimum 1 year of clinical RN experience in an acute healthcare setting
• Current CPR certification

• Strong communication and interpersonal skills
• Ability to work independently and manage multiple priorities
• Comfortable with daily travel between facilities
• Proficient in electronic medical record (EMR) systems
