What You'll Do

Perform comprehensive PT evaluations and build individualized treatment plans
Deliver skilled physical therapy in line with physician orders and agency policy
Track patient progress and adjust care plans as needed
Teach patients and caregivers safe techniques, exercises, and equipment use for lasting results
Document visits and clinical findings accurately and on time
Flag changes in patient condition to physicians and the care team
Provide clinical supervision to PTAs
Take part in case conferences, care planning, and in-service education

What You Bring

Degree from an accredited Physical Therapy program
Active, unrestricted PT license in good standing
1+ year of physical therapy experience
Home health experience a plus, not required
Current CPR certification
Valid driver's license, reliable transportation, and auto insurance
Strong clinical judgment, communication, and documentation skills
