Now Hiring: Physical Therapist (PT) – Home Health PRN

What You'll Do
- Perform comprehensive PT evaluations and build individualized treatment plans
- Deliver skilled physical therapy in line with physician orders and agency policy
- Track patient progress and adjust care plans as needed
- Teach patients and caregivers safe techniques, exercises, and equipment use for lasting results
- Document visits and clinical findings accurately and on time
- Flag changes in patient condition to physicians and the care team
- Provide clinical supervision to PTAs
- Take part in case conferences, care planning, and in-service education
What You Bring
- Degree from an accredited Physical Therapy program
- Active, unrestricted PT license in good standing
- 1+ year of physical therapy experience
- Home health experience a plus, not required
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
- Strong clinical judgment, communication, and documentation skills
