The ideal candidate will have long-term care experience, be compassionate and caring, handle multiple priorities, work flexible hours and motivate staff.**Responsibilities*** Record a patient’s medical history accurately* Take and record measurements of blood pressure, temperature, heart rate etc.* Observe patients under treatment to identify progress, side-effects of medications etc.* Monitor patients’ condition including fluid intake and output and compose patient charts* Assist ailing patients in daily necessary activities such as eating, bathing etc.* Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks* Collect samples for lab testing as assigned* Provide emotional and psychological support when needed* Communicate with patients’ family or friends to provide advice, comfort and release instructionsWhile we are looking for individuals who are dedicated healthcare professionals, we also realize that you need – and deserve – quality benefits for yourself and your family. We offer equal employment opportunity in a drug-free and smoke-free campus## Qualifications**Qualifications*** Graduate of an accredited Practical Nursing program.* Current, unrestricted LPN/LVN license in the state of practice.* Knowledge of long-term care regulations, resident rights, and skilled nursing facility standards.* Experience providing direct patient care to geriatric and long-term care populations.* Ability to administer medications, treatments, and physician-ordered therapies safely and accurately.* Skilled in monitoring residents' conditions and reporting changes to the interdisciplinary care team.* Experience with electronic health records (EHRs) and clinical documentation.* Knowledge of infection prevention and control practices.* Strong assessment, communication, and critical-thinking skills.* Ability to supervise and delegate tasks to Certified Nursing Assistants (CNAs) in accordance with state regulations.* Ability to work effectively with residents, families, physicians, and interdisciplinary team members.* Compassionate, resident-focused approach to care with a commitment to preserving dignity and quality of life.* Current CPR/BLS certification. #J-18808-Ljbffr Adams Farm Living
