Community Health Advocate & Care Navigator

Thrive Alliance in Indiana seeks a Community Health Worker to join a care team, providing education, advocacy, care coordination, and practical support to individuals across homes and community sites. You will work with seniors, disabled, and other clients, documenting services in data systems and coordinating with healthcare providers to improve outcomes and independence.
