Registered Nurse

Position Details Position Title: Senior Long-Term Service Care Manager (RN) Location: Remote Texas - Member-facing Duration: 6-Month Contract (possibility of conversion) Schedule: Monday–Friday, 8:00 AM–5:00 PM Position Summary We are seeking an experienced RN to serve as a Senior Long-Term Service Care Manager. In this role, you will oversee the assessment, coordination, and delivery of care services for complex, high-acuity members with medical and physical health needs. You will develop personalized care plans, ensure members receive appropriate services, and act as a key advocate and resource for members and their families/caregivers. Key Responsibilities Develop, implement, and update long-term care plans for high-risk members. Coordinate care between members, families, caregivers, providers, and community resources. Monitor member status, identify changes in condition, and adjust care plans as needed. Facilitate education for members and caregivers regarding conditions, benefits, and available resources. Conduct home or site visits (monthly or more, as required). Ensure compliance with state, federal, and clinical guidelines through accurate documentation. Partner with leadership to improve service quality and outcomes. May serve as a preceptor/mentor for new clinical hires, supporting onboarding and training. Qualifications Education: Graduate from an accredited School of Nursing required; BSN preferred. Experience: Minimum 2–3 years of pediatric nursing experience required ; 4–6 years total related experience preferred. Case management and Medicaid/home- or community-based care experience strongly preferred. Licensure/Certification: Active RN license (Texas or compact state). RUG/PDPM certification required. Skills: Strong care coordination, case management, and patient advocacy abilities. What We’re Looking For Must Haves: Pediatric nursing background, active RN license, strong care management skills. Nice to Have: Prior case management, Medicaid, or community-based care experience.
