Family Support Navigator — Community Care

Communitas, Inc.

Communitas, Inc. in Wakefield, MA is seeking a Specialized Family Support Coordinator to perform home visits, assess family needs, and identify community supports and DDS options. You will develop a Family Support Plan to promote growth and independence, and assist families in accessing SSI, SNAP, MassHealth, housing, and transportation benefits.
