Remote Bilingual Spanish & Vietnamese Care Coordinator

Alignment Health is seeking a remote bilingual Spanish care coordinator for the SNP case management team. The Care Coordinator works with the RN Case Manager to close care gaps and address coordination needs for SNP members with complex/ chronic needs. Schedule: Monday–Friday, 8AM–5PM Pacific Time.
