Compass Health Network is seeking a Community Support Specialist - Youth in Owensville, OH. The role supports youth and their families in homes and community settings, based on personalized Wellness Plans. You will collaborate with schools and healthcare partners, providing coaching, resources, and crisis support while maintaining regular documentation and client engagement.
