Physical Therapist (PT) - Home Health Visits
PRN/Per Diem | Bay County, FL (Panama City, FL and surrounding)

- Provide one-on-one physical therapy services in a home health setting
- Help patients improve mobility, strength, and balance
- Set and manage care goals that promote recovery and independence
- Evaluate the home environment for safety and mobility needs

What you bring:
- Graduate of an accredited PT program
- Current PT license in Florida
- Valid Florida driver's license, active vehicle insurance, and clear MVR
- 1+ year of experience required, Home Health experience preferred
- Sound judgement and a passion for patient care and outcomes
