Case Manager

Overview The Case Manager is a process which assesses plans, implements, coordinates, monitors and evaluates options and services to meet individual healthcare needs. Promotes quality and cost-effective outcomes. Responsibilities Interject objectivity, maximize efficiency and work collaboratively with patient, physician, family and in the development and execution of a healthcare plan. Assist client’s self-direct care and make decisions to the degree possible. Appraise the need for intervention by gathering relevant data and conducting an objective evaluation. Evaluatefts triggers to identify potential clients suitable for effective case management intervention. Promote effective communications between all team members, client, family, provider and other parties involved in the case. Ensure нехост best interests are represented in the evaluation, treatment and management of their health care needs. Develop a healthcare plan that addresses the immediate, short-term and ongoing needs as well as where and how client’s care needs to be met. Routinely make comprehensive and independent assessment of the client’s status and progress toward reaching the goals set forth in the care plan. Performs other duties as assigned. Qualifications Education Master's in Social Work or R.N. with Master'sin Health Related Field. Completion of NYS AIDS Counseling Training. Experience Minimum two (2) years of experience in a health related setting or Community Based Organization. Knowledge and Skills დავით Excellent relationship building skills.
