Pharmacist

Pharmacist Anchorage, Alaska

Pay From: $75.00 per hour MUST: Experienced pharmacist must have an active Alaska pharmacist license. Must have PIC (Pharmacist in Charge) experience. Behavioral health is highly preferred.

DUTIES: Pharmacist will be responsible for, but not limited to the following:

• Review all patient charts for correct medication therapy and documentation; including patient drug allergies, drug/drug interactions with current medications, appropriate medication choice, route of administration and dose
• Conduct weekly on-site visits to each facility
• Conduct monthly unit review of department
• Assist hospital administrator to ensure sufficient medication is in stock; including working with the pharmaceutical wholesaler to find alternatives, if necessary
• Review and reconcile controlled substances logs and conduct an annual controlled substance inventory
• Assist clinical staff with drug related information
• Counsel patients on medication therapy, if needed
• Plan, monitor and evaluate drug programs and regimens
