Job Description:

Job Summary:

As a Client Care Manager, you will be responsible for managing a caseload of clients and overseeing the delivery of high-quality care services. Your primary focus will be on building strong relationships with clients, their families, and our caregivers to ensure that their unique needs are met and exceeded. You will also be responsible for conducting home visits, assessing client needs, creating personalized care plans, and monitoring client outcomes.

Responsibilities:

• Manage a caseload of clients and oversee the delivery of care services to ensure high-quality, personalized support
• Conduct home visits to assess client needs and create personalized care plans
• Monitor client outcomes and make adjustments to care plans as needed
• Build and maintain strong relationships with clients, their families, and our caregivers
• Collaborate with other team members to ensure seamless coordination of care services
• Identify and address any concerns or issues related to client care
• Maintain accurate and detailed records of client care plans, assessments, and progress
• Participate in ongoing training and education to stay up-to-date on industry best practices and regulations

Qualifications

• High School Diploma required

• Active CNA/CMA license in Nebraska required

• Valid driver’s license & insurance

• Strong computer, communication, and organizational skills

• Knowledge of basic medical terminology

• Bilingual in English & Spanish is a plus
