If so, apply now as a PRN RN with Kindful Health!Job Summary: As a PRN RN of Kindful Health you will work with the Clinical Director to assist a care team including RNCM(s), LPN(s), Aides, MSW and Chaplain to provide best in class care to a reasonably sized case load. The RNCM will attend IDT every 2 weeks with updates from the care team and coordinate with the Clinical Director and Medical Director to assure best in class care. Essential Education and Experience: Must be a graduate of an approved school of professional nursing. Must have at least 2 years of nursing experience in the Hospice service line in the state of Texas. Responsibilities & Duties: PRN RN will be responsible for:Adheres to all regulatory requirements and professional standardsFollows policies and procedures consistent with recognized standards of professional practiceIdentifies system problems or inefficiencies that may negatively affect patient careManage clinical excellence within your patient census.Provide high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scoresCollaborate with your Clinical Director, Medical Director and entire team to provide a "concierge" level hospice experienceQualifications & Skills: The best candidate for this opportunity demonstrates mastery of all facets of hospice care and displays a communication style that makes everyone who works with them want to do their best.
