Job Title: Home Health Aide
Service Location: Canadensis, PA and Effort, PA
Shift: Part-time
Rate: $14-$16/hr
Immediate Need:
Canadensis, PA - 4pm to 6pm Monday through Friday
Effort, PA - Fridays 2pm to 6pm and Saturdays 8am to 6pm

Responsibilities:

• Performs personal care/ household duties (e.g. dressing, toileting, feeding, light housekeeping) in home/school setting
• Maintain a clean and safe environment
• Provide emotional/behavioral support
• Follows plan of care
• Ability to interact as a member of a multidisciplinary team
• Reports any changes in the patient condition to agency/nurse case manager
