This is a meaningful field role focused on caring for patients where they are most comfortable — at home — during some of the most important moments in their recovery.You will manage an assigned panel of patients receiving **acute and transitional care in the home**, including individuals recently discharged from the hospital, emergency department, or skilled nursing facilities, as well as medically complex, homebound patients with lower acuity needs.If you value relationship-based care, clinical autonomy, and the opportunity to truly make an impact, this role offers a rewarding alternative to traditional clinic or facility-based practice.## What You’ll Do* Conduct in-home health assessments and focused physical exams* Manage acute and post-acute conditions for patients transitioning from hospital, ER, or nursing facilities* Develop, implement, and document individualized care plans* Order, interpret, and evaluate diagnostic tests* Prescribe medications and treatments within scope of practice* Collaborate closely with physicians, care managers, and interdisciplinary team members* Educate patients and families to support recovery, safety, and long-term health at home* Maintain timely, accurate clinical documentation## Service AreaThis position serves patients throughout the South Bay, including:* Lawndale* Gardena* Compton* Torrance* Long Beach* Redondo Beach* Hawthorne## Schedule & Compensation* **Schedule:** Full-time, Monday–Friday* **Salary Range:** **$119,312.60 – $149,140.20 annually*** Bonus eligible based on individual and company performance## Qualifications* Master’s degree in Nursing (MSN)* Adult/Geriatric Nurse Practitioner certification* Minimum 1 year of Nurse Practitioner clinical experience (home-based or transitional care experience a plus)* Active, unrestricted RN and NP license in the applicable state* Current TB test or willingness to obtain prior to start**Compensation:**$119,313.00to$178,968.00At CareMore Health, we provide effective Advanced Primary Care and Palliative Care by seeing the whole person — body, mind, and spirit.
