Job Summary:
Tryfacta is seeking a LVN / LPN - LTC for our client in Clifton Springs, NY 14432 . This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!



Position Title: LVN / LPN - LTC

Location: Clifton Springs, NY 14432

Duration: 13 weeks (Possible extension)

Work Schedule: Mid 5x8-Hour (15:00 - 23:30)

Pay rate : $35.00 - $47.00 /hr



Responsibilities for this position include, but are not limited to:

• Provide direct nursing care to adult and geriatric patients in a skilled nursing/LTC setting.
• Perform patient assessments and monitor vital signs and overall condition.
• dminister medications and treatments, including oral, IM, SQ, eye drops, and ear drops.
• Perform blood glucose monitoring and respond appropriately to abnormal results.
• Provide feeding tube, NG/G-tube, and Foley catheter care .
• dminister oxygen and provide basic respiratory care using face masks and nasal cannula.
• Provide skin care, bathing, mouth care, ambulation, and other patient-care activities.
• Monitor patients for changes in condition and promptly communicate status changes to the RN/physician.
• ssist with patient admissions and complete required nursing documentation.
• Maintain accurate patient records using EPIC .
• Follow infection-control procedures, isolation precautions, and PPE requirements.
• Provide age-appropriate care to young adult, middle adult, and geriatric patients.
• Collaborate with RNs, physicians, pharmacy, respiratory services, radiology, social services , and other members of the interdisciplinary team.
• Serve as a charge nurse when assigned , coordinating patient care and supporting nursing staff.
• Float to other units within the LPN/LVN scope of practice as required.

• ctive New York LPN license required in hand at submission .
• Minimum 1 year of experience required .
• BLS certification required .
• EPIC experience is a hard requirement .
• Previous charge experience required .
• LTAC experience required.
