Assignment Duration

This is a 6-month contract position beginning September 28th.

Job Description

The Outpatient Physical Therapist is responsible for evaluating all assigned patients and developing, implementing, and modifying treatment plans according to the patient's needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that enable effective return to community activities.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Complete medical record documentation in a clear, concise, accurate, and timely manner.
• Ensure care is provided under signed, valid physician orders and within insurance approvals.
• Participate in student programs and provide a positive learning environment.
• Actively create and pursue an individualized professional development plan.
• Adhere to discipline-specific State Practice Act and other regulatory agencies.
• Demonstrate awareness of organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1 year of outpatient clinical experience is required.
• Current CPR certification is required.
• Active and applicable license for the state where work will be performed is required.
• Experience with Select Medical or previous agency assignments is highly desired.
