We are hiring Full-Time or PRN PTs to provide high-quality care to residents in our skilled nursing and post-acute rehab programs. You will work closely with nurses, physicians, and other rehab staff to create and implement individualized treatment plans, ensuring residents receive the best care possible. Key Responsibilities Conduct comprehensive evaluations to determine therapy needs Develop and implement individualized treatment plans Provide therapy interventions to improve mobility, function, and independence Educate residents and families on exercises and treatment plans Document progress and adjust therapy plans as needed Collaborate with healthcare team for care coordination and discharge planning Ensure compliance with CMS and facility documentation requirements in NetHealth EMR Maintain knowledge of evidence-based best practices in physical therapy Education & Requirements Associates Degree or higher in Physical Therapy from an accredited program Active New Jersey Physical Therapy license Ongoing commitment to continuing education and professional development Long-term care experience is a plus Strong organizational, communication, and patient care skills Why You’ll Love Working Here At Crystal Lake Healthcare & Rehabilitation , we understand that skilled therapists have many options.
