Occupational Therapist (OT)We're seeking a skilled and compassionate Occupational Therapist (OT) to provide high-quality care to patients in home care and hospice settings. This is an opportunity to make a meaningful impact by helping patients improve their independence, comfort, and quality of life.ResponsibilitiesConduct comprehensive evaluations of patients' functional status and home environment.Identify equipment and adaptive needs.Develop and communicate personalized treatment plans in coordination with physicians and care teams.Teach and retrain patients in daily living activities, adaptive equipment, and prosthetics/orthotics.Provide specialized care to hospice and home care patients.Maintain accurate clinical documentation and report changes in patient condition.Communicate with care team members and supervise therapy assistants as needed.Educate patients, families, and caregivers on treatment plans and therapeutic techniques.Participate in team meetings and assist with discharge planning.RequirementsDegree from an accredited occupational therapy program.Registration with the National Board for Certification in Occupational Therapy (NBCOT).At least 2 years of clinical experience as an Occupational Therapist.Hospice and home care experience preferred.Strong verbal, written, and organizational skills.Current CPR certification.Valid driver's license and reliable, insured vehicle.Current Occupational Therapy License.Benefits401(k)401(k) matchingDental insuranceHealth insuranceVision insurancePaid time offFlexible scheduleWork Location: Laconia, NH / On the road
