Rapid Transition Housing Specialist

Exceptional Family Resources in Syracuse seeks a Housing Specialist to support individuals facing homelessness or housing vulnerabilities. This role involves assisting participants in securing suitable housing and coordinating essential support services. The ideal candidate will have a high school diploma (college degree preferred), solid communication skills, and experience working with individuals with disabilities. Responsibilities include responding to referrals, working with community partners, and maintaining documentation.
