Care Coordination & Referral Specialist

You will facilitate referrals, schedule appointments, and coordinate travel and lodging in line with SEARHC policies. The role emphasizes strong customer service, data entry into EHR systems, and prompt communication with patients to ensure timely and smooth care coordination.
