Nurse Practitioner or Physician Assistant

Job Summary : Choptank Community Health System (CCHS) is seeking a highly motivated and skilled Nurse Practitioner or Physician Assistant to work in our School-Based Health Center (SBHC) program. SBHC provides an effective means for students to access comprehensive health care services. SBHCs provide care in a location that meets students where they are: at school. Multiple studies have shown SBHCs to be a cost-effective investment in child and adolescent health by reducing the utilization of emergency rooms, improving health outcomes, and elevating school attendance and graduation.

Located in the heart of Maryland’s Mid-Shore region, Choptank Community Health System (CCHS) provides primary health care services in Caroline, Dorchester, Kent, Queen Annes, and Talbot counties. Maryland’s Eastern Shore enjoys the beautiful Chesapeake Bay setting, characterized by historic towns, meandering creeks, beautiful natural areas, Maryland Blue crabs, oysters, and clams. All this, and we’re still just a short trip to Baltimore, Washington, DC, and Philadelphia.

Our Joint Commission Accredited system includes six medical offices, five dental offices, over forty medical and dental providers, behavioral health, school-based wellness, and migrant health programs, for a total of 200 team members. Our mission is to provide access to exceptional, comprehensive, integrated health care for all.

This is an exempt, full-time position in pay grade 9 with the pay range of $94,056.00 - $141,083.00. The SBHC Nurse Practitioner or Physician Assistant   reports directly to the Community Based Program Director. 

Education and Experience:

• Maryland License as a Family Nurse Practitioner or Physician Assistant

Working Conditions and Physical Requirements:

• General office environment in clinical health centers
• Occasional travel 
• Frequent exposure to workplace conditions includes bending, climbing stairs, walking, kneeling, and reaching. 
• Includes fine and gross motor skills, lifting less than 20 pounds, and vision.
• Occasional exposure to workplace conditions includes air borne pathogens, blood borne pathogens and cleaning agents.
