About the Role:

As an LPN at Spring Creek Rehab, you will play a vital role in delivering high-quality patient care in a comprehensive rehabilitation setting. Your responsibilities will include:

• Providing direct patient care, including administering medications, wound care, and treatments as prescribed by physicians
• Developing and implementing individualized care plans in collaboration with patients, families, and healthcare teams
• Conducting patient assessments, monitoring progress, and reporting changes to nursing staff and physicians
• Collaborating with interdisciplinary teams to ensure seamless transitions and continuity of care
• Maintaining accurate and detailed records, including patient charts and documentation
• Participating in quality improvement initiatives and educational programs to enhance patient outcomes and nursing practices

Ideal Candidate:

• Must be a Licensed Practical Nurse (LPN)
• Strong clinical skills and ability to prioritize patient care
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment with a high level of attention to detail
• Commitment to providing patient-centered care and contributing to a positive work environment
