Job Summary Support regional outreach efforts by coordinating external relations, helping coordinate and deliver outreach, education, and engagement initiatives to improve access to care, health literacy, and population health within a defined geographic region. Provide support to the regional outreach department through a wide variety of duties to ensure services are provided in an effective and efficient manner.

Performance: Position Specific Essential Functions
- Work with appropriate management and staff to effectively facilitate problem resolution related to issues raised by outside providers regarding patient referrals and services.
- Communicate issues to appropriate staff and follow-up on any decisions or actions taken.
- Communicate changes to external providers in an appropriate and timely manner.
- Provides administrative support to ensure efficient office operations. Answers phone calls and direct callers to appropriate personnel.
- Responds to emails and other digital inquiries or correspondences.
- Drafts and edits correspondence, reports, and other documents.
- Inputs and updates information in databases and spreadsheets.
- Prepares meeting agendas and takes meeting minutes.
- Coordinates logistics for meetings, including room setup (and catering when applicable).
- Research as requested, and compiles and summarizes information for reports or presentations.
- Completes projects within assigned timeframes and enlists support from appropriate internal and external resources, when appropriate.
- Works closely with other administrative staff and supports other colleagues as needed.
- Works with sensitive information with discretion to maintain confidentiality and security and ensure compliance with privacy policies and regulations.
- Ensures that deadlines are met and adapts to changing priorities.
- Performs other duties as assigned.
