Physical Therapist (PT) Location: San Francisco Compensation: $60–$63+/hour DOE Schedule: Full-Time, Part-Time, or PRN City View Post Acute is seeking a compassionate and motivated Physical Therapist (PT) to join our exceptional in‑house rehabilitation team. Key Responsibilities Conduct comprehensive resident evaluations and develop individualized, evidence‑based treatment plans. Deliver skilled physical therapy services focused on improving mobility, function, and quality of life. Collaborate closely with nursing staff, Occupational Therapists, Speech Therapists, and interdisciplinary care teams. Educate residents and families on home exercise programs, mobility safety, and long‑term injury prevention. Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards. Monitor resident progress and adjust treatment approaches as appropriate to support successful outcomes. Qualifications Current California Physical Therapist (PT) license or eligibility for licensure. Strong clinical reasoning and communication skills. Passion for geriatric rehabilitation and patient‑centered care. Collaborative, positive, and team‑oriented approach.
