RN Hospice Field Visits - Nights

Trustbridge Hospice, part of Empath Health, is seeking a compassionate and dependable Registered Nurse (RN) to provide rapid response overnight field visits throughout Broward County for hospice patients & families in need of immediate support. This role is ideal for a nurse who thrives in independent practice, stays calm under pressure, and is driven by a sense of purpose, delivering skilled, responsive, in‑home care during overnight hours 7:00pm –7:30am and every other weekend. What you'll Do As an RN with Trustbridge Hospice, you will play a vital role in ensuring patients and families feel supported, informed, and cared for during urgent and often emotional situations. Support and uphold the organization’s mission, vision, and values in every interaction Coordinate patient and family care in collaboration with the attending and hospice physicians Perform comprehensive patient and family assessments to identify physical, emotional, and psychosocial needs Develop and manage the Interdisciplinary Group (IDG) Care Plan Ensure timely implementation and coordination of all interventions and services outlined in the care plan Respond promptly and effectively to emergent needs and concerns during field visitsli> Deliver a consistently high level of personalized, compassionate, and engaging care Advocate for patient comfort, dignity, and quality of life at all times Foster trust and reassurance to ensure the highest level of patient and family satisfaction Why Join Empath Health? What You'll Need RN degree from a college or university, accredited by the Accreditation Commission for Education in Nursing and/or the Commission on Collegiate Nursing Education. Registered Nurse currently licensed in the State of Florida. Current Basic Life Support Certification. Basic computer skills and EMR software experience a plus. Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
