PACE Place provides coordinated medical and social services to elders who want to remain in their home but need support to do so. PACE Place provides Medical, Rehabilitation, In-Home Assistance, Transportation, Social Work, and Day Center services for patients in our care. Position Summary The PACE Center Manager oversees and manages the daily operations of the PACE Day center, including transportation, CNAs, activities, and dietary care. This includes ensuring that appropriate personnel are trained and perform their functions within the organization according to operational services standards. Must also ensure budgetary criteria are met, and operational compliance as per governing regulations. Assume responsibilities of the Executive Director’s in his/her absence. Essential Functions Oversee the daily coordination of the PACE Day Center As a member of IDT, continuously evaluate the participants' changing needs and ensure necessary program adjustments. Works and collaborates with center peers (including IDT, Clinical Manager, Enrollment & Community Outreach, Home Care Coordinator and Quality) to determine best practices and approaches for the PACE Partners. As a member of IDT, participates with coordination of care for participants to include home health, Registered Dietician, and medical/specialty services according to participant care plans. Assist in providing in-service education for the PACE Place staff. Supervise and evaluate the performance of direct reports. Ensure continuity of day center operations. Responsible for maintaining compliance with state and federal program requirements for the day center to include maintaining day center licensure, food safety and sanitation, and CLIA Assist in program preparation of PACE Place’s short-range and long-term plans, budgets and related information systems based on broad organization goals and growth objectives. Participate in all interdisciplinary team meetings and serve as facilitator as needed. Support the interdisciplinary team’s care management process to ensure best decisions are made regarding the participant’s changing condition. Assists, as necessary, in the development and adherence of the annual operational and capital budgets for departments of accountability and managing services to meet financial benchmarks. Assist Human Resources Manager as needed in the PACE Place’s operational policies, procedures, protocols, and regulationsfor direct participant care and support services. Assists and collaborates with Human Resources Manager on employee performance, employee relations, investigations, discipline, and other staff needs. Monitor operations to ensure consistent compliance with established quality assurance standards. Performs recruiting duties and collaborates with Human Resources and Recruiting partners on staffing needs for the PACE Partners. Recommend changes to Executive Director in service delivery and staffing as required due to the development and growth of the organization. Participate regularly in all quality assurance/quality improvement committees as part of the overall quality improvement program. In conjunction with the Executive Director, maintains the stability and reputation of PACE Place by ensuring that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing, and certification requirements governing operations. Monitor operational compliance with Occupational Safety and Health Administration (OSHA) regulations and agency policies and procedures. Protect privacy and maintains confidentiality of all company procedures, results, and information about, employees, participants, clients, or families. Completes all required compliance and continuing education classes and/or training programs and events. Comply with all state and/or federal regulations to ensure the documenting, tracking and monitoring of ordered, care planned or approved DME, home delivered meals, and supplements. Comply with all state and/or federal regulations to ensure the organization is performing assessments, collecting and analyzing the data from these assessments to take actions to improve the quality of the participant’s life. Monitoring and oversight of the IDT to ensure unscheduled reassessments by the IDT are being conducted as required per all state and/or federal regulations. Comply with all state and/or federal regulations to ensure the IDT coordinates 24-hour care delivery that meets the needs of each participant across all care settings. Comply with all state and federal regulations to ensure that all ordered, approved, or care planned services are provided to the participant. Comply with all state and/or federal regulations to ensure all complaints regarding quality of care or service delivery are properly processed as a grievance. Comply with all state and/or federal regulations to ensure all individual issues within a grievance are reviewed, considered, and resolved. Comply with all state and/or federal regulations to ensure immediately approved SDRs have notification of the approval during the same encounter and/or interaction. Comply with all state and/or federal regulations to ensure all participant medical records are complete, accurate and accessible to all staff. Maintain professional affiliations and any required certifications. Provide administrative on-call coverage on a rotation basis. Only act within the scope of his or her authority to practice Perform extra or unrelated duties as assigned. Qualifications Knowledge and Critical Skills Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently. Either training, or related experience in direction of a health care program Knowledge of the physical, mental, and social needs of the elderly Valid Florida Driver’s License, required Active Auto Insurance coverage, required ACHA Level II background, required BLS Certification required Experience A minimum of one-year of staff supervisory experience in a social services or health care services setting, and a minimum of one-year of experience in providing services to the elderly, including dementia Must have 1 year of experience working with a frail or elderly population OR, if the individual has less than 1 year of experience but meets all other job requirements, the candidate must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring. License / Certification ACHA Level II background, required BLS Certification required Education A minimum of a bachelor's degree in health care services, social services, nursing, or a related field Other Requirements Employees must have medical clearance for communicable diseases and have up-to-date immunizations before direct participant contact.
