Per Diem Extended Hours Clinical Manager (RN)

Position Summary

VNA Health Group is seeking an experienced Registered Nurse to serve as a Per Diem Extended Hours Clinical Manager. This role provides clinical oversight and support during evenings, weekends, holidays, and other non-business hours to ensure continuity of care for Homecare patients. The Clinical Manager serves as a resource for field staff, assists with urgent patient care needs, and helps ensure quality outcomes and regulatory compliance.

Responsibilities

• Provide clinical support during evenings, weekends, holidays, and other extended hours

• Serve as a resource for homecare field staff

• Triage and address urgent patient care concerns

• Collaborate with physicians, patients, families, and team members

• Support continuity of care and appropriate service delivery

• Monitor quality of care and clinical outcomes

• Support regulatory compliance and documentation standards

• Communicate significant clinical issues to leadership as needed

Qualifications

• Current NJ RN License required

• BSN required

• Minimum 2 years of home health/community health experience

• Leadership experience preferred

• Knowledge of OASIS and home health regulations

• Strong communication, organizational, and clinical decision-making skills

• Valid driver's license and reliable transportation

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD 40 to USD 52 per hour.
