Job Title: Licensed Practical Nurse (LPN) MDS Coordinator
Location: Camellia Health and Rehabilitation | Mobile, AL
Licensed Practical Nurse (LPN) MDS CoordinatorCamellia Health and Rehabilitation is seeking an experienced and detail-oriented Licensed Practical Nurse (LPN) MDS Coordinator to join our clinical team in Mobile, Alabama. This position plays a vital role in ensuring accurate resident assessments, timely MDS completion, regulatory compliance, and quality reimbursement while working collaboratively with our interdisciplinary team.

ResponsibilitiesComplete and submit MDS assessments accurately and within required timeframes
Coordinate the MDS process with the interdisciplinary team
Develop and update resident care plans based on assessment findings
Ensure compliance with CMS, Medicare, Medicaid, and state regulations
Review clinical documentation to support accurate reimbursement
Monitor assessment schedules and submission deadlines
Participate in quality improvement initiatives and survey readiness
Communicate effectively with nursing staff, therapy, physicians, and families as needed
Current Alabama Licensed Practical Nurse (LPN) license in good standing
MDS experience in a skilled nursing or long term care setting preferred
Knowledge of PDPM, Medicare, Medicaid, and the RAI process
Strong assessment, documentation, and organizational skills
Excellent communication and computer skills
Ability to work collaboratively with an interdisciplinary team
