The role requires collaboration with patients and families in both internal and external settings. It involves a comprehensive assessment of patients' goals as well as their biophysical, psychosocial, environmental, functional, and financial status. Responsibilities also include procuring services and advocating on behalf of patients and families for scarce resources. The RN Case Manager directs families to arrange for services that require patient and family decision-making and participation. Acting as a liaison to post-hospital care providers and community health resources is a key function. Continuous communication with patients, families, medical and hospital staff, caregivers, and post-hospital resources is essential. Facilitating or providing necessary patient and family education prior to admission, during the hospital stay, and after discharge is required. The role involves developing and maintaining a positive work climate and positive working relationships with all stakeholders. Essential job functions include assessing patients' healthcare needs and developing comprehensive care plans. Facilitating communication between patients, families, and healthcare providers is part of the responsibilities. Coordinating transitions between care settings, ensuring continuity of care throughout the treatment process is vital. Evaluating medical necessity for treatments, procedures, and hospital stays is a required function. Collaboration with insurance companies to obtain authorizations is necessary. Monitoring length of stay and identifying potential discharge barriers are included in the duties. Ensuring the appropriate use of healthcare resources while advocating for patients' needs and preferences is essential. Educating patients and families about diagnoses, treatments, and self-care is part of the job. Connecting patients with community resources and support services and addressing barriers to care is included. Regular reviews and updates of care plans based on client progress are necessary. Facilitating referrals to specialized services, advocating for clients with service providers, and representing client interests within complex service systems are part of the responsibilities. Maintaining accurate and thorough case management records is expected. Documenting assessments, interventions, and outcomes, along with tracking quality metrics and patient outcomes are required tasks. Preparing reports for healthcare teams and administration, and working with other healthcare team members is included in the role. Participating in interdisciplinary care conferences and consulting with specialists regarding complex patient needs is important. Coordinating care plan implementation across disciplines is a required duty. The employee must support the hospital mission, vision, values, policies, and procedures, and participate in required education as applicable to the position. This position requires sufficient auditory ability to detect and respond to various sounds, including patient alarms and verbal communication. Effective communication is essential, as is the ability to interact with patients, families, and healthcare team members. Visual acuity is necessary for observing patient conditions, reading documentation, and distinguishing colors for clinical purposes. Tactile perception is important for assessing physical characteristics during patient care. The role requires motor coordination for performing fine motor tasks necessary for the use of medical equipment and procedures. Occasional stooping, kneeling, crouching, or crawling when assisting patients or accessing equipment is expected. Walking and standing are typically continuous throughout the shift. The position requires the ability to lift up to 50 pounds independently and involves frequent pushing, pulling, carrying, or repositioning of objects or patients. There may also be occasional lifting or transferring of individuals weighing over 100 pounds, with the expectation of using appropriate techniques or team assistance for safety. This position is performed primarily in an indoor healthcare environment, such as a hospital or clinic. The work setting may be subject to variable indoor temperatures and involves frequent exposure to infectious and contagious diseases. There is an expectation for all staff to follow appropriate safety protocols and utilize personal protective equipment as required. The healthcare environment can be fast-paced and unpredictable, requiring the ability to respond quickly to changing circumstances, including high-pressure or emergency situations.
