As a Mental Health Social Practitioner (MHSP), you will support individuals living with serious mental illness (SMI) through a recovery-oriented, community-based model rooted in the Clubhouse philosophy—where "community is therapy."About Pacific Health Group Pacific Health Group is committed to delivering innovative, compassionate, and community-driven care. Our Clubhouse Program provides a structured, empowering environment where members and staff work side-by-side to build skills, foster independence, and support long-term recovery.Minimum QualificationsBachelor's degree in Psychology, Social Work, Rehabilitation Counseling, or related field (preferred)Experience working with individuals living with serious mental illness or related populationsStrong understanding of mental health practices, recovery models, and trauma-informed careExperience with Medi-Cal documentation and behavioral health systems preferredExcellent communication, interpersonal, and engagement skillsStrong organizational and time management abilitiesMust be proficient in technology, including documentation systems and Microsoft Office (Outlook, Teams, Word, Excel)Knowledge of Diversity, Equity, and Inclusion principlesBilingual preferred but not requiredWhat This Role Looks Like (Day-to-Day Reality)Work side-by-side with members in daily Clubhouse operationsFacilitate structured programming within a "work-ordered day" modelBuild meaningful relationships with members and support recovery goalsSupport employment, education, and life skill developmentParticipate in community engagement, events, and outreach activitiesMaintain real-time documentation and reportingThis role requires strong emotional intelligence, adaptability, and comfort working in a collaborative, member-driven environmentMember Engagement & SupportBuild and maintain meaningful, supportive relationships with membersOrient new members to the Clubhouse model and available servicesSupport members in achieving goals related to housing, employment, education, and wellnessProvide advocacy and support during challenging or crisis situationsProgram Coordination & Daily OperationsFacilitate daily Clubhouse activities aligned with the work-ordered dayWork collaboratively with members to operate program functionsCo-facilitate meetings and support structured programmingSupport Services & Resource NavigationConnect members with housing, benefits, and community-based resourcesSupport members in daily living skills and community integrationAssist with behavioral health engagement and service coordinationEmployment &Education SupportSupport transitional and supported employment placementsAssist members with job readiness, applications, and trainingFoster relationships with employers and education providersCommunity Engagement OutreachBuild relationships with community partners, employers, and stakeholdersParticipate in social, wellness, and outreach activitiesRepresent the Clubhouse program within the communityDocumentation & AdministrativeMember engagement and participation in Clubhouse activitiesProgress toward member goals (employment, education, housing, wellness)Timely and accurate documentationStrength of relationships with members and community partnersContribution to a positive, recovery-oriented program environmentHow Success is MeasuredWho Thrives in This RoleIs passionate about mental health recovery and community-based careBuilds trust easily and connects with diverse populationsThrives in collaborative, team-based environmentsIs proactive, compassionate, and solution-orientedIs comfortable working in dynamic, people-focused settingsWork RequirementsAbility to work on-site full-time in San Diego locationsAbility to participate in occasional evenings, weekends, or eventsValid California Driver's License, reliable transportation, and current auto insuranceAbility to attend required Clubhouse training and program-related trainings (conducted within California)
