Key Responsibilities\n \n Conduct comprehensive assessments and provide therapy through various modalities, including individual and group therapy sessions, tailored to meet the diverse needs of clients. \n Develop, implement, and evaluate personalized treatment plans, ensuring alignment with best practices and organizational protocols. \n Utilize interviews, case histories, and observational techniques to gather pertinent data about clients' mental health conditions, analyzing this information to inform treatment decisions. \n Maintain a manageable caseload, offering both individual and collateral therapy to foster holistic care. \n Perform quality assurance checks on client documentation, ensuring that all records, including assessments and progress notes, adhere to clinical standards and regulatory requirements. \n Actively engage in case consultations and team meetings, providing insights and collaborating on care strategies aimed at enhancing client outcomes. \n Adapt and refine assessment and treatment protocols per health plan regulations, ensuring the implementation of best practices. \n Patient Population / Client Focus\n Your work will primarily focus on clients within the mental health spectrum, including those facing significant life challenges and emotional distress. In this position, you will support individuals navigating difficult transitions, such as those associated with hospice care, grief, and emotional trauma. Your expertise will be essential in providing compassionate, evidence-based therapeutic interventions. Qualifications\n \n A Master’s Degree in Social Work from an accredited program, ensuring a strong theoretical foundation in social work principles. \n Active and unrestricted California Clinical Social Worker License (LCSW). \n A minimum of 2 years of experience working in a healthcare setting, with at least one year in mental health. \n Proven clinical experience in case management or direct patient care within a social service or healthcare context. \n Demonstrated ability to effectively manage conflict and communicate with diverse patient populations. \n Preferred Experience\n \n Familiarity with electronic health records (EHR) systems and digital documentation practices. \n Experience in delivering care to clients with complex health needs, particularly in hospice or end-of-life settings. \n Training in trauma-informed care approaches. \n Compensation and Benefits\n The compensation for this position reflects our commitment to attracting high-caliber professionals.
