Position Summary This Monday–Friday Physician Assistant role offers independence, purpose, and balance. As part of our rounding group, you’ll visit patients in local senior living communities, deliver consistent care, and help keep them safely at home. Responsibilities Provide primary care and medical follow-up in senior settings Participate in care planning and transitions Collaborate with office support teams and nursing staff Maintain clear, timely documentation Qualifications Licensed PA-C in Minnesota Experience in geriatric or home-based care preferred Compassionate, dependable, and clinically strong Vehicle and auto insurance required Military Occupational Classification (MOC) Codes Army: 65D Navy: PA Air Force: 42G1 Marines: PA Coast Guard: PA Join Our Team Enjoy meaningful work with time left for life outside of work.
