Position Type: Full-time, PRN

Salary Range: $45-$55 per hour, depending on experience

Work Environment & Population Served

Our organization operates Skilled Nursing Facilities (SNFs) that offer:

  • Long-term care for residents needing continuous skilled support and compassionate daily assistance.
  • Short-term, sub-acute rehabilitation for individuals recovering from surgery, illness, or injury.
  • Collaborative care through a team-oriented approach involving nursing, therapy, and medical staff working in unison.
  • A diverse patient demographic, including those with orthopedic, neurological, cardiopulmonary, and post-acute needs.

The patient mix may vary slightly by facility. As a therapy professional, your role will be to assist patients in achieving functional improvements and enhancing their quality of life within a supportive, patient-focused environment.

Position Summary

The Physical Therapist (PT) delivers direct rehabilitation services in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

Education, Experience, and Licensure Requirements
  • Graduation from an accredited physical therapy program (BSPT, MSPT, or DPT; those graduating in 2003 or later must hold an MSPT or DPT).
  • Current registration or licensure in the applicable state, with an obligation to maintain an active license in good standing throughout employment.
  • Preferred: At least one year of experience in post-acute care or a related setting.
  • CPR certification required upon hire or to be obtained during orientation; must remain current throughout employment.
Specific Job Responsibilities
  • Engage in community health initiatives and projects as appropriate.
  • Proficient in Microsoft Word, Excel, and email communication.
  • Collaborate with patients, families, and support departments to effectively plan for patient needs.
  • Exhibit proper body mechanics consistently.
  • Make independent decisions when necessary.
  • Familiarity with practices, procedures, and regulations governing post-acute care facilities.
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures.
  • Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
  • Maintain professional relationships with colleagues, vendors, and others.
  • Safeguard the confidentiality of all proprietary and sensitive information.
  • Adhere to organizational policies, including those related to harassment and compliance.
  • Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training.
Essential Functions
  • Implement physical therapy treatment plans for patients.
  • Set, evaluate, and adjust realistic, measurable, and functional goals.
  • Supervise and assess care provided by PTAs, Rehab Aides, and PT and PTA students.
  • Document care accurately and in a timely manner.
  • Utilize therapy software effectively and accurately.
  • Provide excellent customer service and maintain a positive attitude towards patients.
  • Assist in the safe evacuation of patients when necessary.
  • Demonstrate reliable and consistent attendance.
  • Employ reasoning skills and sound judgment in decision-making.
  • Communicate effectively and collaborate within an interdisciplinary team.
  • Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours.
  • Read, write, speak, and comprehend the English language.
