What You'll Do

• Conduct patient screenings and evaluations for cognitive and ADL physical functioning disorders.
• Develop and implement individualized plans of care following regulatory and clinical standards.
• Utilize standardized assessments and evidence-based practices to support effective interventions.
• Administer therapeutic activities, exercises, and modalities to assist patient recovery.
• Provide patient, family, and caregiver counseling related to physical disorders.
• Document all services and patient progress in electronic health records accurately.
• Collaborate with health team members and participate in care conferences and trainings.
• Supervise and support occupational therapist assistants, rehab aides, and students.

What You Need

• Current Occupational Therapist licensure (as required by state and regulation)
• Experience working in long-term care or similar clinical settings preferred
• Strong communication and patient care skills
• Ability to work well within a multidisciplinary team
